Opens our HIPAA-secure form in a pop-up. Use your client’s information, then continue to Step 2.
Fax your referral details

- Whether the client agrees to the referral, whether a release of information is on file, and whether you’d like ongoing care coordination
This form is not for emergencies. If your client is in crisis, call 911 or have them call or text 988, the Suicide & Crisis Lifeline.
